Application Checklist
Ph. D. Program: Ph.D. study involves working closely
with one or more mentors, therefore in addition to the
standard application procedures, prospective doctoral
students should contact members of the department whose
work is of particular interest. Ph.D. applications
are considered for the Fall and Spring Semesters. Our
deadline's for accepting Ph.D. applications are January
15th for the Fall semester and October 15th for the
Spring semster.
Au.D. Program: Professional Doctoral Degree is a clinical
degree program in Audiology. Au.D. applicants are accepted for fall entry only. Students interested in a
non-clinical master's program in audiology may apply
for other semesters.
Masters Program: Students applying for the clinical
masters program in speech language pathology are
accepted for fall entry only. Those desiring non-clinical
programs in speech and hearing science may be admitted
for other semesters.
Our application is online at the Graduate
school web site. Paper applications are no longer

The required grade point average is 3.0 or the equivalant
of a B on your undergraduate or post gradute coursework.
Official reports of Graduate Record Examination (GRE)
scores from tests taken within two years of the date
of planned entry into a program are required of all
applicants.

International Applicants are required to take one of the following:
TOEFL iBT; (A 27 is required for the Speaking portion of the test and an overall score of 77 is required to be admitted) or
TOEFL (620 minimum score or 260 for computerized test)
TSE (50 or above)
TWE (no minimum)
OR
IELTS (A score of 7.5 is required)
ALL documents must be received by January 15th. All documents go directly to the SLHS Department. If you use the application system to send recommendation letters they are accessed by the graduate secretary and do not need to be mailed.
An application fee of $55 is required of all applicants.

Purpose (300-500 word essay). This form will print out with the application. There is no need to send it separately.

GRE scores: List Purdue University at West Lafayette as one of the destination schools
or we will not receive your GRE
scores and your file will not be complete. International
students must also take the TOEFL, TWE and TSE
exam and have the scores reported to the Purdue
University. If you prefer you may use the IELTS for you speaking score instead of the TSE. A speaking score is required along with the TOEFL and written scores for English.

- Documents that you must ask others to send:
1. Three letters
of recommendation. It is helpful to give the
recommendation forms to your reference writers
at least 14-21 days before they must be sent
and to attach a prominent note to remind the writer
of this date (we suggest December 15th).
2. Official transcripts
from every college or university that you have
attended. If your fall grades are not available
to be on your transcript by late December, have
the available transcripts sent and inform us of
your grades. International transcripts should
be sent in your native language as well a translation
in English.

School website.
Please note any other names that you have used
in school on all documents sent. We receive documents
with different names on them and have no way of
connecting them unless you tell us.
