Research in Absentia Form - Please print and complete the form according to instructions. Contact Delayne Graham (
dkgraham@purdue.edu or 496-9629) for assistance.
You will need to submit this to Delayne in order to proceed with the process.
Request for Appointment of Examining Committee - This form is to be used as a DRAFT to setup your Special Field exam or Final Dissertation Defense exam. Please print and complete the form. Y ou will need to submit this to Delayne AT LEAST THREE (3) WEEKS PRIOR TO DEFENSE DATE in order to proceed with the process. Contact Delayne Graham ( dkgraham@purdue.edu or 496-9629) for assistance.
Form 17 - Travel Request: Please print this form and submit to Susan Curtis for approval. She will then direct you to the IDIS Business Office. You will need to file the form with her AT LEAST TWO (2) WEEKS PRIOR TO THE DATE OF TRAVEL .
