Registrar's Form 40 and Instructions

The Registrar's Office has two separate versions of the Form 40: 1) one for undergraduate courses numbered 001-499; and 2) one for graduate/(dual) level courses numbered 500-699. Click "Registrar's Form 40 and Instructions" above to be directed to the Registrar's Office website to obtain forms and instructions.

Please read the instructions carefully to be sure submissions are complete. Incomplete or improper submissions might need to be returned to the department, delaying approval.
